Please fill out this form to reserve equipment per item, per date. Media Services will receive your request as an email. The request will be submitted as long as you receive the proceeding confirmation page upon submission, regardless of whether or not you receive a receipt by email (as this email only works with LATTC email accounts).

Media Services will get back to you via email as soon as possible regarding your request.
