The state's Program and Course Approval Handbook (MS Word) has everything you need for forms and instructions to complete the forms.  This document is over 100 pages -- you should note the page number you are interested in printing, and use the "Print" command in the "File" menu.  Do NOT press the print button on your menu bar, as this may set the entire document to print.
