All curriculum forms are available in both MS Word/Excel and PDF formats. Word files can be opened with MS MS Word or later versions. Excel files can be opened with MS MS Word or later versions. PDF files can only be opened with Adobe Acrobat Reader 5.0 or later versions. If you don't have the free Acrobat Reader, get it here:
Clicking on the title of the form (i.e., "Course Change Request) will open the page in this Handbook describing the purpose and process for completing the form. Forms may be printed on white paper unless otherwise indicated.
Clicking on the application link (i.e., "MS Word") will open up the file in the associated application, which must be loaded on your computer.
Click on the links below to SEE the forms, however, to be able to save what you type, follow these steps:
1. RIGHT click on the name
of the form.
2. Click on "Save Target As..."
or "Save Link As..." (depending on your system).
3. Save the form onto a floppy disk, or on
your hard drive (make a note to remember where you saved it!).
You can then go to that location on your hard drive or floppy to work on your Program Review document.
Clicking on a "zipped" file will open your decompression software, such as Winzip; you must expand the file before you can type on the form.
